If diagnostic examinations and the review of medical records suggest that more serious hospital interventions are needed for the treatment of arrhythmia, the patients will be referred to the contracted hospitals of the Tehran Arrhythmia Center for follow-up treatment. The medical team, along with the trained EP nurses, will perform the necessary hospital procedures such as catheter ablation, cryoablation, or pacemaker implantation to treat patients.

Catheter ablation

Catheter ablation is one of the main and subtlest hospital procedures for the treatment of cardiac arrhythmias. In this method, several fine electrical wires (catheter) are guided to the heart through the veins of the groin. At the end of these wires, there are electrodes for recording the electrical waves inside the heart, which are shown to the doctor on the computer screen through connections as an electrical tape in the control room. This operation is a team procedure and the success of the operation depends a lot on the experience and expertise of the medical team and operating room technicians. In this procedure, by moving these wires inside the heart cavities, the doctor found the arrhythmia center and destroyed these centers using heat waves transmitted to the tip of the wire. With this method, the arrhythmia is completely cleared and the patient returns to a normal life.

3D mapping (CARTO)

Due to the complexity of the heart structure, in some types of arrhythmias, it is necessary to use 3D ablation to find the heart of the arrhythmia and burn it effectively. For this purpose, the engineers and doctors of the medical equipment companies have designed special devices that are capable of simulating its electrical and anatomical structure by simultaneously recording hundreds of electromagnetic or electrical waves from the heart. With the help of this simulation, the doctor locates the arrhythmia center more accurately and directs the ablation wires in a direction that burns the arrhythmia center more effectively and more stably. Sometimes, in this process, an engineering team is added to the medical team so that this simulation is done in the best and most accurate way possible.

Currently, the latest model of the 3D CARTO machine made by the American company Biosense Webster has been installed in Nikan Aqdasiyeh Hospital and the 3D machine Precision (EnSite Precision) made by the American company Abbot has been installed in Diy Hospital. These devices with advanced capabilities have an important application in the treatment of complex cardiac arrhythmias.

Cryoablation

Cath Lab of hospital are now equipped with the facilities and new technology of treating arrhythmias by freezing foci of arrhythmia. With this method, new horizons have been opened in the treatment of patients with irregular cardiac arrhythmias (atrial fibrillation).
In addition to this, patients who were exposed to damage to the natural conduction pathways of the heart due to the proximity of the arrhythmia location and the need for a pacemaker can be well treated with this method. In the cryoablation method, unlike the usual method where the heart of arrhythmia is destroyed by heat and burning, this time the heart of arrhythmia is destroyed through cold damage caused by nitrogen gas. The advantage of this method is that, unlike burning, the chance of damaging the normal tissue of the heart is much less with the freezing method.
